Chapter Reports
Kappa Delta Pi requires chapters to submit annually a series of reports to track chapter operations and chapter activities. The reports help KDP Headquarters ensure that chapters are meeting the minimum requirements and are operating effectively. Your responses to the reports help the Chapter Services staff to provide meaningful support customized to your chapter’s needs. We appreciate you taking time to complete these and return them to KDP Headquarters by the deadlines provided.

Every chapter must submit a list of officers with their contact information for the current school year. If changes occur during the year, an updated form must be submitted. Updates must be made in the Chapter Management Section.

Data typed into this form will not be saved. Please print your completed form, and mail or fax a copy to KDP Headquarters.

Different sections of this report must be completed by various officers, but the Vice President compiles the information and submits the report.

All reports must be submitted each academic year by the chapter. Failure to submit reports may result in ineligibility for awards, withholding or delays in sending chapter rebates, delays in communication with chapter officers and counselors, or chapter suspension.
